Programs & Services

The GRACE Foundation offers programs and resources for individuals and families affected by ASD. Our programming includes, but is not limited to, Day Habilitation, Site Based Respite, and Community Habilitation.  You can learn more about our services below.

Our Programs

Day Habilitation

Our Day Hab program includes scheduled activities, formalized training,
and staff supports to promote skill development for the acquisition,
retention, or improvement in self- help, socialization,
and adaptive skills. Our program runs Monday through
Friday Program for young adults 21 and above.

Respite Program

GRACE site-based respite is so much more than you can imagine,
with activities including performing arts, science, dance, history,
Club Live, Dinner Club, bowling and more!

Community Habilitation

Community Habilitation is a Medicaid-Funded program operated under the OPWDD
that provides 1:1 service or training to individuals with
developmental or intellectual disabilities to encourage
independency in their daily living.

How Do I Qualify For Services?

  The Front Door is a program created by New York State Office for People With Developmental Disabilities (OPWDD) The program connects you to the services you may be seeking out.  Contact your local Front Door Regional Office by clicking below or calling 866-946-9733 to check your eligibility and get started.  Once you are determined eligible, you can begin the planning process. There are many service options available, including support to live in your home independently or with others, employment, day habilitation, children’s services, amd more.
